All jumpers under the age of 16 years must provide a completed and signed Declaration of Informed Consent by a parent or legal guardian, and must be submitted prior to the first session.

The Declaration should be completed online and is valid for all sessions for a period of 12 months after the date of signing (unless there are changes in the declared circumstances (including details concerning health condition), in which case a new declaration must be completed.
